Don't think you will have broken his ear drum but get him to a doctor ASAP. if his ear is bleeding at all then there is a problem. May be nothing but it may need medical treatment.

First of all you are not supposed to clean a child's ears out. If the paediatrician See's that the wax in the ear is or can cause a problem they will remove it. Your child will probably be fine if it was only a small amount of blood like on the q-tip. Is he acting fine? You should probably call the paediatrician on call and ask them before you do anything else. They will call you back and tell you if they think your child should be seen
1)since I'm an RN and I work in pediatrics, I would highly suggest going to the ER or some type of urgent care center
2) you do not use q-tips on a small child/infant.period
Yes that is a sign of a broken ear drum. I would go to the doctor asap. The broken ear drum should heal, and some hearing loss is possible, but it should heal and be fine.
