Just tell your new doctor who your old doctor is, sign a release, & the new doctor will send for them.
when you register with a new doctor, your records are normally automatically sent there by the local health authority. new systems coming in called GP2GP where thay are transferred electronically the same day as registering.

It's amazing to me the amount of misinformation given out in these types of forums. The Health Insurance Portability Accountability Act of 1996 indicates that your previous physician may in fact share your records with your new physician for Treatment, Payment or Other health care operations or TPO. All you new physician need do is request them from the previous physician. Many clinics and hospitals require a signed, valid release of information form. Sign one at your new doctors office, they will send to the old physician.
