The doctor can not give out any information with out your permission. It is a violation of Federal law.
Why should a bank need any health info about you? That makes no sense.

U can inquire about that at your doctors office and they should be able to give u a copy or u may be able to find something online about HIPAA too.
Why did you send the student loan people information on a doctor's visit? As far as I know, a BANK has no right to even request this information from you. You have to sign a piece of paper, giving the doctor's office permission to release this information to ANYONE other then another medical professional who needs the information to care for you otherwise.
