What can cause swelling in the ankle and lower part of the leg and the two smallest toes to hurt.?

The cause of the swelling could be from the pelvis, just like pregnant women get swellings too. For the pain I suggest trying something stronger than paracetamol, try ibuprofen. Good luck.
Naturally you need to see a physician regarding your concerns and let him diagnose you properly. This could be many different things and it's almost impossible for anyone to say without looking at you and getting a full assessment. I believe that you don't seem like you trust the first doctor has done for you. If that is the case how about seeing another doctor for a second opinion? He can actually see and give physical assessment and any other tests if necessary. A physician can rule out determine what is going on much quicker than we ever could. With that said, just for chit chat and just some thoughts and I only recommend you go by what your doctor says and not my blog! But just being inquisitive, and well since I'm here I might as well ask... did you injure your foot a while back? Possibly twist it? Could you have been bitten by something? Did you cut yourself on something, scratch or puncture by and animal or object? -addressing a staff infection. Do you swim in the ocean and perhaps brushed against some coral? (that's a stretch but ya never know!) Did you drop something on your foot? Are you experiencing any other symptoms? If I gently pressed on the top of your foot that it would leave an indention from my finger? -- checking for pitting edema. What motion gives you the most pain? Is the area behind your leg swollen, red and warm to touch? Does your skin "ooz" with fluid? Well, I suppose you see how hard it is to know what is wrong. Wish I could be of more help! If you were my family I would have you see another doctor and get a second opinion and inform him of the medications you are on now. Bless your heart! Sorry you have to experience this! Arthritis can also cause these symptoms. I would recommend getting another opinion but from a rheumatologist (doctors that diagnose and treat diseases and disorders of the joints and bones such as arthritis) to see what they think.
GREETINGS, you need to find out why it's swelling, what specific tests have been done? you need blood work to check for organ functions, gyno exam to make sure there are no cysts blocking or pressing on lymph nodes, blood and urine to test for diabetes, and a veinogram to rule out clots and vein problems, an ultrasound on the groin and leg to check for problems (can be mri or cat scan too).
