A puncture injury may cause some leakage of blood, usually (not much, unless you hit a superficial blood vessel),some internal leaking and swelling and possible damage to the underlying structures. There would certainly be a risk of infection. In general, scrotal puncturing is not a good idea.
The punctured boy should seek medical care immediately, and would probably be encouraged to elevate the scrotum on a pillow, take an anti -inflammatory (like ibuprofen) and apply an ice pack wrapped in a towel, be asked about their latest tetanus shot, and perhaps be given a course of antibiotics.

Lol, I bet all the guys are holding themselves when they read your question. Anyway, I don't know but I used to go with my dad when they cut cattle. They would cut the outer flap of skin then reach inside and pull out the testicles then cut them off and throw them in a box (for dinner, yum yum). They would take a big swab thing and dip it in some anesthetic then dab the calf they cut with it. Not very bloody at all that I recall, the "mountain oysters" were horrible tasting though, all squishy, I projectile vomited them across the table and they never made me eat them again. (Does NOT taste like chicken).
um if it leaked anything it would be blood
