Can a doctor charge a fee for filling out physician statement forms?

Question:
This is not workmans comp related.

Answers:
Many doctors office are adopting this practice if it is many forms to complete for the patient. We tried to adopt the policy but it wasn't fair across the board. They can charge $5-?. whatever they choose to do. So the answer is yes.
I think they would, unless it is part of a physical examination.
