If you go to the doctor for a procedure and don't use insurance (or have insurance), is there a record of it?

is there a "permanent medical record" on everyone, like your credit report, even if you're uninsured? How does it go from one doctor to the other if you don't tell the next doctor about it?

Answers:
Each doctor must keep a detailed medical chart on each patient. As far as I know, there is no "permanent medical record" on everyone, like a credit report. There is no way for it to go from on doctor to the other, if you do not authorize transfer of the records.
Every doctor or nurse has to keep records ---it's the law and in the uk they are duty bound to tell your family doctor
In USA if you pay cash the only records are with that doctor unless you have a communicable disease then the CDC is alerted and thus the county you reside for possible quarantine and mandatory hospitalization.
The other docs get records from insurance and also if you request to share this info
