Does an American have to pay to see a doctor in Canada if they are visiting or living temporarly?

Question:
How does it work? Is it free? If not would you have to pay something? How much?

Answers:
Yes, you'd have to pay. How much depends on where you are (clinic, doctor's office, hospital emergency) and what you are having done. Fees are usually lower than in the U.S.
It would be the same as if you walked into an American doctor. Canadian health insurance only covers citizens of Canada. But...it's not a bad reason to change citizenship.
