Question:
My right knee has been hurting for years. If i bend it for awhile and then try to extend it i can't. Also when i sit down for awhile with my knees bent i can't put pressure on my knee when i get up and it won't fully extend. I had an MRI and x-rays done on it and nothing showed up. The doctor suggested physical therapy again. I went through physical therapy several years ago for it, but it didn't seem to help. It swells up all the time, i have a warm sensation under my knee cap and the back of my knee hurts badly when i walk. Does any one know what's wrong with it? Or, have you had similar injuries? I'm debating whether to try PT again, have the doc perform surgery in case there is something that didn't show up, or get a 2nd opinion.

Interesting problem. It sounds like you may need to get a second orthopedic opinion, repeat the MRI with more views and most probably have an arthroscopy which will be definitive. This is a small keyhole operation and is probably the gold standard. It sounds like a meniscal tear, however this should have been picked up by the MRI. Therefore I would suggest:
1. second opinion
2. Repeat MRI and proceed
3. Consider arthroscopy.
