my understanding is that you have one family physician, this one may be different from the one you had as a child, as in finding your own family physican. Your medical records are ususally faxed or sent over and any appropriate documentation would be there. It would be possible that each would not know of each other if you happened to have 2 family physicians. It could also create a conflict of interest too for them both. It would be interesting to see what OHIP would think on this matter as they in essence would be double billing. I think if you paid for every aspect of the care OHIP would not mind.
The only other time you usually have a different family doctor is if you are admitted in the hospital and a hospitalist is appointed for you. This helps to maintain practice as well as not all family phsicians have hospital privileges these days.
