damn...alot of people i know said there best way was cold turkey(and yes they smoked as many years as you)...and now you just got to avoid people who smoke, out of sight out of mind, right..good luck bro.
It's like any addiction. First get rid of ALL of it and all of the stuff you use to smoke it including ashtrays, papers, or whatever. Then avoid it like it was the plague. Don't hang out with people who smoke it. Most importantly, find a substitute activity for the times you would smoke. Take up knitting, guitar playing, water color painting, whatever. Substitution tends to be the most effective way to get rid of an addiction.
getting away from other people that smoke helps & stopping the urge at its roots by cutting out your ways of obtaining so called "herb". However, its mainly getting into the right frame of mind and staying there. Once your a cronic smoker its hard to get away from it unless your really dedicated. You even start dreaming about it. It is a gateway drug. not a "fall in the well drug".
I wont tell you to start other drugs, because those will likely be even harder to quit, even though cocaine has no physical/chemical addiction.
Go to rehab and treat with good health care.
You should get rid of items that you use to smoke it - the actual cannabis, ashtrays, pipes, etc. Maybe you could try looking for girlfriends who don't smoke it. If you get girlfriends who do smoke, chances are that you are going to be influenced by them.

DO NOT SMOKE IT. No matter what. Just be strong. Believe in yourself. Trust yourself that you know what you are doing.
