My friend is 22 and she is still covered under her dad's health insurance. However, she plans to visit a clinic for women's health with a personal problem she does not want her family to find out about. It is a clinic that helps lower income people (although anyone may go.) All she'd have to do is show proof of income (she has a job) and the clinic will decide what percent she pays. Is it legally acceptable for her to choose to not have the visit covered by the available insurance, but instead to pay in cash, since she wants the matter to be kept private from her family?

Yes. She can certainly do that. She's 22, so she can make her own decision, and I don't blame her for wanting to keep things private from her family!
