Yes. If the nurse believes it is not required to see the doctor you will still be billed. However, you should tell the nurse that despite her or his feelings you still want to see the doctor and they will let you see the doc.
Always ask to see the doctor and they will stop this crap.
KNITTING F. is correct. It is common practice for a nurse practioner or a physicians assistant to see cardiac patients. These people are usually well trained in specific fields, and have an M.S to PH.D level education. In spite of this, you do have the right to see the cardiologist, as well. If you are uncomfortable with the former, don't hesitate to make your wishes known. The NP or PA should not object to this request. You are the patient, and you are the boss,too. You pay these people to take care of you, so to speak, and you have to look at it this way. Should you get objection, I would stay shy of anyone but the cardiologist. No one should be insecure enough to dissuade you from seeing who you want to see. Also, if the cardiologist gives you any flack about seeing him instead, get yourself another Doc. Don't allow any disrespect, from anybody, doctors included.
