TMT shows significant ST changes in leads 2,3, AVF & v4 to v6. Explain. Postive for exercise inducible ischemi

This isn't really a question, but a test result. What is shows is a heart that has decresed blood supply that causes changes in the EKG, which is the electrical output of your heart with exertion.
The marks on an EKG are labelled P-QRS-T. The P is the contraction of the upper chambers of your heart, the QRS is the contraction of the larger lower chambers of the heart, and the T is the relaxation phase of those lower chambers, called the ventricles. The lead numbers and letters represent directions through the heart, and which ones have changes show the cardiologist what areas have decreased blood supply. "ST" changes are the place in the PQRST the hungry muscle shows up. "Ischemia" means dimininshed blood supply.
This usually means the coronary arteries which feed the heart blood have a blockage or other restriction of flow, from clots, build up of plaque or other damage.
More tests are needed to determine what kind of problem exists. A nuclear scan would show whether muscle has already been permanently damaged or if it is possibly reversible. The "gold standard" would be a heart catherteriztion to map the exact problem, and even fix it with a balloon or by placing a stent.
A test result like this one indicates a need for more follow up and whoever has this needs to promptly follow up with a cardiologist, preferably an interventional one. It's not a time to panic but to take necessary action.
Hope that answers some of your question, but this should be discussed with the doctor until you are satisfied with what it means.

If after the stress is over the changes are totally reversed,Ischemia is confirmed.
Sounds like a decreased blood supply to mainly the inferior portion of the heart. If you're not clear on your test results, you should go back to your doc and have him/her explain them to you.
