well let me tell u about acute coronary syndrom:
a clinical syndrom characterised by ischemic cardiac chest pain characterised by ST segment change & T wave changes but ST segment elevation as seen in the clasical acute myocardial infarction..
there is elevated cardiac enzymes as troponins I & T(which are normally not present in the blood), but may be elevated in some other conditions as renal problems,dermatomyositis etc... CK is not specific for cardiac damage, so measurement of CK MB is preferred..

the clasic ischemic cardiac pain is: restrosternal, compressive rather than pain, might radiate to the arm, neck or jaw. comes on exertion, lasting less that 10 min. (if 30 min = MI) reliefed in 2-5 min after rest or nitroglysrin intake....

heart failure: if the paient is found to have it, u must search for the cause...
types: right sided, left sided, congestive heart failure..
the cause can be classified: due to pressure over load (as systemic hypertenssion or aortic valve stenosis on the left side or pulmonary hypertension or pulmonary valve stenosis on the right side) or due to volume overload (as mitral valve regurgitation on the left side or tricusped valve regurge on the right side)
congestive as when the right side failes as a result of left sided heart failure)...
