Okay, what's your question? Your telling me that an elderly man is having chest pain, and difficulty breathing. Sounds like a Heart Attack, is it sudden onset or chronic? Any history of Cardiovascular problems, either personally or in his family? Has any nitro been given to lesson the pain (given of course he does not take Viagra) . What is your question though?

could be a heart attack doesnt have to be painful. may be congestive heart failure are lungs filling , retention of water weight loss etc.
Call 911, if he is on nitroglycerin and is also having chest pains give him one of those until the paramedics arive.
