after controlling my diet and taking allapurinol daily, still have acute gout attack twice a month. pls help!?

There are 2 aspects of managing gout.

One is pain relief. Gout pain is caused by inflammation. Of all the anti-inflammatory drugs, colchicine is usually most effective against gout inflammation. Other anti-inflammatories help (arcoxia is one example - there are many others). Try to keep active, and distract yourself from thinking about pain. The pain will go away, but may come back. Stopping pain does not get rid of gout.

The other aspect of managing gout is long-term prevention. That is why you are taking allopurinol. It is taken to block production of uric acid. Too much uric acid forms crystals and these crystals get attacked by your immune system. This causes the inflammation that I mentioned above. The big problem is that your immune system cannot kill uric acid crystals - it just hides them. The good thing about allopurinol is that it will eventually get rid of the excess uric acid and you will become gout free. There is a side effect to this, and it applies to anything that reduces uric acid, not just allopurinol - fixing the problem can cause short term pain.

As soon as the drug starts to work, uric acid levels fall and existing uric acid crystals start to dissolve. You cannot avoid this - you need to stop crystals building up before they cause bone and cartilage damage. Unfortunately, when they start to dissolve your immune system sees them again and starts another attack. It is a common problem with any gout cure - the period when you are getting better can mean more gout attacks until all the uric acid deposits have dissolved. This can take months, even years, depending on how long you've had gout.

Drinking lots of water throughout the day will help. Reducing the allopurinol dosage may help, but will prolong the period it takes to get rid of the deposits. You must not change this without discussing it with your doctor.

There are no easy answers, but if you persevere, the gout will go eventually. Get your doctor to keep a check on your blood uric acid levels. This will fluctuate, but you need to aim below 6mg / dl to stop new crystals forming and get rid of existing crystal deposits.

A number of people I know have had great success in relieving and even eliminating gout by having a reflexologist do deep, forceful massage of their feet. This apparently actually breaks up and dislodges the crystals quite well. They say it hurts when you're having it done, but that afterwards they had no discomfort at all. You might give it a try. My friends were most enthusiastic about the process and their success. This may sound a little strange, but many people have gotten total relief from eating cherries, and/or drinking cherry juice. Some chemical in the cherry is said to take care of the problem.


There are other treatments out there for acute gout,

Colchicine is one of the medications that is effective in reducing the pain, swelling, and inflammation associated with acute gout attacks. The pain often subsides within 12 hours of starting treatment, and is completely relieved in 48 hours.
