How do I take care of my grandpa if he has a really high fever?

He needs an enema ASAP. Go to the drugstore and buy him some douche.

That said, You could try putting cool washcloths under his armpits, and on his forehead and on his groin area. Have him take some tylenol. That should bring it down pretty quick. Change the washcloths every 15 minutes with cool ones.
