I am suffering low back pain up to right leg,after MRI i have been told my problem is l4 l5 and l4 s1 so what

Also i feel pain and sometimes numbness, but i can walk as normal man , so my concern is when one doctor o\f orthopedic told me that i need surgery, and another two doctors orthopidic and neurologist told me , no , that i dont need a surgery , just only a physio therapy, so i am seeking your advice.

PS:

I am suffering the leg pain for almost 20 years, but now gratually i felt a back pain.

It sounds like what is termed as "degenerative disc disease"-meaning that your discs (cushy material between each vertebra) have deteriorated overtime and that perhaps they are not as "cushy" as they used to be. Your spinal nerves are housed inside the vertebra and there are certain points along each vertebra where those nerves exit and continue out to your body. The disc keeps the bones above and below from "pinching" those nerves and causing pain, numbness and even paralysis to the affected part of your body. Over time the vertebra themselves deteriorate and can shift out of place(again pinching) the nerves. Without knowing more it's hard to say if one of your disc is bulging (herniated). Often times physical therapy can help to "realign" the spine and allow discs to reposition properly so as to cushion the vertebra in your spine. Sometimes discs can also rupture and the result is no cushion between two vertebra and hence the nerves are significantly pinched between two bones. Surgery (usually microdissection) is done to repair the injured disc or in some cases to remove the fragments left from the rupture(very irritating to the nearby nerves) and fuse the two affected vertebra together so as not to pinch the affected nerves. Always try PT first to see if it helps vs immediate surgery. Deterioration of bone over time is not something that can be changed with surgery anyway. My ex had microdissection of his discs at L4-L5 and has never had a pain free day since (3 1/2 yrs. later) so not all surgery is successful. There is one bright spot though-a procedure called "artificial disc replacement" is now approved by the FDA and people are having good results. It's something my ex is considering vs having spinal fusion of his affected area. Hope this helps!

Yep hurt but if the majority says no to a very serious surgery that could possible leave you in a worse place then try the easiest solution first. My husband injured his l3 and l4 and did a chiropractor and does not wear a wallet in his backside anymore and got a firm mattress. Other doc wanted to slice him then and there. Go with less evasive first.

Try the conservative treatment the neurosurgeon asked for. If it doesnt work, then go back and see about surgery. There are things you may not be familiar with that happen with surgery. It is not a first choice anywhere unless needed!
