So the question is:

Would you rather die from throat/lung cancer, coughing up blood, attached to an oxygen bottle everyday, not being able to do anything physical, possibly disabled until you die. Also, because of smoking, all of your close friends and family have been poisoned by the smoke, and you're to blame.

~OR~

Slowly losing your mind, amnesia, motor function loss, inability to feed yourself and (almost) needing to wear a nappy.

I would choose a bullet to the Medulla Oblongata. It's quick, painless and much less horriffic than either of these two. Plus (with regards to the first option) I won't poison my friends and family in the process.
