Now, to the question: a low HDL cholesterol is usually genetic. Families with low HDLs tend to develop coronary artery disease and those with high HDLs tend not to do so. In fact, there is an Italian family with an especially good form of HDL that runs very high in their lineage; it is being studied through recombinant techniques and has been shown to actually reverse existing atheroma when infused intravenously.
