Peritonitis (infection of the abdomen) or sepsis (whole body infection of the blood), would set in probably within an hour, or depending on the size of the rupture a few hours at most. Then you would spike a fever, your heart rate would rise as your breathing increased, your body's immune system would go crazy trying to fight the infection and finally your blood pressure would drop as your heart began to fail and you would go into septic shock and die. All the while having INTENSE pain in the belly.
