DId the doctor send the fluid to have it examined for malignant cells and did he do a ct scan to examine the chest? Send your mother to an oncologist for 2nd opinoin. Pleural effusions can mean many things.
It doesn't matter which side the effusion is on in regards to cancer. It could have been in her belly and still been an issue r/t her old breast ca.

One of the defining properties of cancer is its ability to metastasize. Therefore ALL cancers are metastatic. However, that does not mean it is guaranteed to metastasize, but it has the ABILITY to do so. Growths that cannot metastasize are called benign tumours.

Being estrogen receptor positive means the doctor has another option for treatment via hormone therapy (ie Tamoxifen). It does not indicate how well it will respond to treatment.
