You need to seek professional help (i.e. podiatrist).

Other Answers:
loceryll nail polish, available in 3 and 6 ml. Apply once a week The treatment takes up to 6 months depending on the severity of the nail infection.

Clotrimazol or Bifinazol ointments( active ingridients) are also effective against nail fungal infection.
The nerve endings sound like there affected, & a Dr. can give you something for this. But, in the mean time, use Lanocaine.
I don't think you should still be suffering from nail pain if the infection is totally healed. Either there is still a fungal infection making the nail thick and thus causing an ingrowing toenail or you may be suffering from a different nail related condition. Ingrowing toenail happens when the edge of a toenail grows to extend into the skin surface causing pain and often bacterial infection. If your painful symptoms persist, the best thing to do is to see a dermatologist (skin specialist doctor).
