extremely good question. there is at least two types of prevention.
if we talk about primary prevention we mean prevention before the occurance of the disease. secondary prevention is all about prevention of repeating the scenario or the complication of the disease.
most research has done on the secondary prevention. but i think you are talking about the primary prevention. so it is not very easy to answer your question in a scientific way. but i simply refer to the following point.
the best measure is just keeping your blood pressure at normal level. because it is a known cause of stroke.
keep your cholestrol level at normal (LDL most important)
quit smoking if you are a case
reduce your weight
if you are a diabetic case, keep your blood glucose in the normal range
aspirin or any other similar drugs have not been preved to be effective in primary prevention
exercise is very very important at it lowers blood pressure, it reduces the need to insulin, it improves perfusion, and a quite a lot of other good things!

All of the above are good suggestions. My mother died of a stroke at age 70. Her doctors had told her for several years that if she did not get her blood pressure under control, she would probably have a stroke.
She did not control her BP, for one reason or another, and she proved them right. Almost all other health factors have an influence on BP, and it is the main cause of stroke. So Keep a handle on BP, and you definitely have a leg up. Live Long And Prosper! Balance diet, exercise regularly.
But if the elderly is not used to exercise, then try to keep him/her entertain at all time.
Beside blood pressure problems, one issue that we need to look into is elderly's brain functioning.
I think many people have neglect this problem.
Try to give them some things to do, make their brain entertain but not too stress. With all these, the elderly will not have time to think too much of negative issues which can somehow contribute to the heart and health burden.
One best solution, as the younger generation and children, try to spare some time for them. Accompany from people can destress and this will contribute to illness-free at old age.
