Perhaps it's because the fungus is usually attached to or inside macrophages. Without a proper T-cell response, you don't get activation of your monocyte derived cells, so you don't as strong of a proliferation compared to a person with a functioning immune system. Complete shot in the dark.

Update: I just realized that HIV also attacks macrophages because they also contain CD4. So, no macrophages equals no infection with blastomycosis.
