Assuming you are talking about cancer.
A cancer in any locations can be either primary or secondary. Primary means it originates from that site. Secondary or metastasis means the cancer starts elsewhere and migrates to the current place. For example, liver can have primary hepatocellular carcinoma or cholangiocarcinoma. Liver cancer is however, often secondary especially when there are numerous lesions in the liver. The histology or cell/tissue appearance can be obvious or subtle requiring special stains in determination of the primary site. The liver may simply show adenocarcinoma, squamous cell carcinoma or poorly differentiated small cell or non-small cell carcinoma. Further study is necessary to identify the primary site such as possible metastatic colonic adenocarcinoma or breast cancer spreading to the liver. Not uncommonly, after thorough testing, the primary site remains undetermined or unknown primary!!
The oncologists can still treat the patients with chemotherapy and/or radiation based on the pathology report despite unknown primary.
