There is a good reason why diltiazem may interact with grapefruit juice, in that diltiazem is a substrate for cytochrome PYP3A4. Grapefruit juice inhibits the activity of this enzyme in the gut wall, leading to potentially higher blood levels of diltiazem when taken at the same time as grapefruit juice. Having said that, there have been a few small trials that showed no significant effect.
The simple answer is to be on the safe side, and avoid the combination.

yes, but you should take it with a full glass of water instead whenever possible.. No problems will be caused with absorption when taken with grapefruit juice.
