I have usually headache and forget things very soon impaired talking behaviour some time become blank .?

minor headach very difficult to remeber the things and loss of confidence and for the last 3 month i have observed a round shape protrudtion in right upper limb and vomiting during emergency like exams and all.

If you were older I would say you were having a transient ischemic attack also known as a mini-stroke. Since you are relatively young I'm not sure if that guess is appropriate unless you might also have a brain tumor or other serious neurological impairment. In any case you need to be seen by a physician who takes your complaints seriously as soon as possible since you may have a very serious problem going on.
