This could be any number of things. How about calling your doctor's office and asking for a diagnosis so you know what to research? After all, that's part of what you pay for when you go to the doctor.
well vasculitis is inflammation of arteries and you can have this in head causing pain...
Try looking up muscle spasm of the cervical spine or nerve impingement of the cervical spine. Muscle spasms in that area of the body are called torticollis. Good luck.
Given your description and the doctors recommendation of physical therapy, my guess is that you have a "pinched nerve". This where the neural tunnel is in someway compromised by the muscles and tissues that surround it. If this is the case, the doctor is right with physical therapy. Loosen the muscles that affect the impinged nerve. You can do more by doing daily stretching exercise especially to the affected region (neck and shoulders).
Please remember that this can become serious with blackouts and dizziness. I have fallen to the floor with an episode of "pinched nerve" in my neck. Be very careful please. Oh yeah, and don't stress so much...a common cause of tight muscles.

It actually sounds like your physician does not have a good handle on why you are having this pain (or else they should have given you a diagnosis).
Most likely, he/she is thinking this may be related to some type of mechanical disorder...meaning some type of neuromusculoskeletal tissue is being irritated...either due to an altered joint position, tissue tension, or poor posture.
I highly recommend you see a therapist with MDT experience. These are PTs who are experts in treating people who have mechanical neck and back pain. That will probably be more likely to get you some difinitive answers. I recommend going to http://www.mckenziemdt.org to find a credentailed provider...you can also find out more about the term "mechanical pain."
