46 is not too young at all for serious heart disease.

If my doctor had responded to my first chest pain 6 years ago at age 47 or 48, I may not have needed a triple bypass.

My heart was/is very strong, but the arteries were so nearly totally blocked at age 53, that once I had angina, I needed an emergency triple bypass. Three weeks before, my regular doctor said I had anxiety. He nearly killed me. My blockages were 99%, 99%, and 80%. I had 5 minutes of angina in his office, and the EKG registered nothing wrong.

You may just as easily be having unstable angina for a near total blockage. The thallium test can't identify the severity of the blockage. I had one of these a year ago. They put in a stent, sent me home, and I had a heart attack in the area of the stent less than 24 hours later. They then put a 2nd stent an inch away from the first one. My cholesterol was HDL 47, LDL 137. Now they are HDL 45, LDL 61.

You need to be on a VERY low fat diet. As for anxiety, well, be happy each day you wake up, its better than the alternative. Sounds like you get the most anxiety from people who don't want to treat you appropriately. Write your will, and relax and make fun of those folks will be arguing about the fact they thought you just have anxiety.

No alcohol, no smoking. If you feel like a walk, take one. Not long, not fast, not uphill. Enjoy each day like its your last, but behave as if you'd like to have 100 more. Stop disaggreeing with the wife, unless she is one of those that disagree with everything.

Just say, "Ok" a lot. If you're dead tomorrow, no point upsetting anyone today.

If you are Hindu, threaten, with a laugh, to come back and taunt them if they are not nicer too you. THAT would be the sweetest revenge on those disagreeable folks around you.
Your list is too long and you are too young to be going through this. My doctor would have suggested dancing and a vacation. That is what he suggested to me when I had angina pain just by going through a sewer back up in my house.
I hope this helps you.
To Help with the attacks have you tried meditation?
Reaffirm your "safeness": I am safe, my loved ones are fine, there is nothing wrong here.
Take a few deep breaths, put a nice warm cloth over your nape of the neck. Sit down and recite a favorite poem, till the feeling leaves. It may take some time at first.
Listening to soothing music sometimes helps the feelings pass.
Watch your caffeine level, coffee, chocolate, soda pop, that can trigger episodes at times as well.
I wish you well, and I hope this can help you.
