Answers:
In addition to playing chess a lot, you should try studying it. Get a book or two, and study games that have been played by chess greats. Don't feel bad about a 5 game losing streak, you will come out of it eventually. 5 games isn't really even that bad. You might want to focus on other things for a while if chess is making you feel insane. Maybe go work out. That will distract you from chess for a while, but will get blood flowing to your brain more efficiently, thus helping your game in some small (or quite big) way. It will also cause your brain to produce mood boosting hormones that will make you feel better. Other than that just play a lot and study the game. Good luck!
Find another hobby to master. You are burnt out on chess. If you have such passion try learning to play an instrument or something.
It's a good habit, so don't be hard on yourself about it. Try to play with some different opponents; that will diversify your game.
Are you talking about concentration or insane..
This belongs in the psychology section, but nevertheless it just seems like you just put too much pressure on yourself. Everyone "used to be" better than what they are now at something... ever hear an old person tell stories about "how they used to could (blahblahblah) like you could?" They're not wasting their breath so just practice some more. It's just like everything else... the less you do it the worse you'll be. It takes a while to get back good again. Nevertheless don't develop such an obsession with it. It's just a game, and your just a player ;) Like life, for instance.
