74 yr old male with a cough that happens at every meal when he takes the first bite and?

Ok. What is it that he does with that first bite? Is he so hungry that he eats it too fast? Maybe trying to swallow before he's really chewed it well? Maybe he should try to drink something right before that first bite---just to prepare his throat. Also, he can try something very soft first---like soup.
Another idea----is he on medication that causes him to have dry-mouth? If he doesn't have a lot of saliva, that could cause that first bite to get stuck. You usually get more saliva the more you eat during a meal. I take blood pressure medication and that causes me to have dry mouth a lot. I always drink something first---just to get some moisture in there before I take that first bite. After the first bite, I'm fine.
Look on the bright side-----they could not find anything wrong. That's great! Try some of those ideas and see if they help.

That sounds alot like my father in law. He has a "pocket" in his esophagus. Having surgery on it soon.
