They are the same only in a healthy heart.Heart rate is the rate at which the heart actually beats where as pulse rate is the rate at which we FEEL the pulse on the radial artery(in the wrist).If cardiac contraction happens to be very weak it may not produce a pulse.So,in such cases the pulse rate is less than the heart rate.

Yes, it occurs in most of the time.
However, you may appreciate difference between heart rate and pulse in cardiac arythmias like Atrial Fibrillation in which you may appreciate your heart rate is higher than the pulse rate.It is due to the fact that not all impulses(abnormal impulses) produced in the atria,transmitted to the ventricle as,the ventricular contraction clinically produces our pulse rate.
In other clinical circumstance called"pulseless electrical activity" of the heart where, you may notice a perfect heart rhythm on the ECG monitor without a pulse.It means that , your heart is having some form of electrical activity but, not producing any cardiac output(Blood is not ejected).
Hence, theoretically,in a normal individual ,the heart rate should shadow the pulse.But, in clinical situation, you may appreciate a lot of variation in this matter.
