It's called "terminal restlessness"
There are conflicting opinions as to whether this is caused by physical issues, like from the meds, organ failure, pain, lack of oxygen to the brain and/or generalized infection
resulting from medication toxicity, organ shutdown and the associated metabolic changes, pain, urinary or fecal retention, dyspnea and related hypoxia, and sepsis; or if it's caused by psychological stress and awareness of dying.
It's very hard to watch.
