My grandma says to run hot water on it, which is going to make it itch bad (while doing it), but bring some relief afterwards.

Having had poison ivy a number of times, I would encourage you to not itch it at all, as it will get the liquid from the sores under your fingernails and it spreads wildly...anywhere your fingers touch will end up with poison ivy, including other people.
the ONLY thing I have found that will help me is an ointment called: Triamcinolone Acetonide cream USP 0.1%
It will stop the itching, it will halt the spread and dries it up like nothing I have ever seen. Available by prescription, here at least...
