Knee Replacement Surgery?

Question:
Knee Replacement Surgery - High Risk Candidate
Risk Factors: COPD/Emphysema (Oxygen 24/7's), Sleep Apnea (Bi-Pap Mach.), Atrial Fibrillation, Age 70 & Over Weight 50+ lbs. Had previous back surgery of ruptured disk but no fusion's. Anesthesiologist is suggesting a Spinal w/Mild Sedation, Nerve Block in groin area ... w/scare tissue in the lower back which could create problems for a Spinal. Have met with the Orthopedic Surgeon and Anesthesiologist & want to schedule surgery in October.
Appreciate advice/opinion of someone in the know.

Answers:
I have been an orthopedic nurse for 25 years and have seen countless knee and hip replacements. You are actually very typical of the patients I see. Spinal anesthesia is a great choice even without other medical problems, its much easier on your body especially your lungs and heart, not to mention the fact it allows for a period of no pain after the surgery. Your anesthesiologist will determine if the block or spinal is best for you. I think you will be happy with the surgery, I have had so many patients come back later telling what a difference it has made in their lives.
If the anesthesiologist suggests a spinal, then he/she probably doesn't feel too worried about the scar tissue -- another option would be to do a sciatic block -- it's a nerve that runs off of the spine into the butt, and down the leg -- I don't know if it would block everything, though -- you'll have the check with the anesthesiologist.

It's strange how, with so many things that could go wrong with the cutting and bleeding and pounding, we focus on the lack of sensation.
The docs are doing knee replacement surgeries using spinals and sedation. My friend had it a few months ago, and he's fine.
I had a total knee replacement and I too had spinal surgery. Had 2 herniated discs and a lamenectomy. I have scar tissue and would not have a spinal only because I'm a coward and was afraid I would feel too much pain. Yeah, I'm a nurse, but a coward when it comes to anyone touching my spine. Probably because I have 5 more herniated discs and refuse surgery. However, if the doctor is suggesting a nerve block and spinal with sedation, he obviously knows what he/she is talking about. I would get another opinion. Never, ever take one Md's opinion. I usually get 3 and if 2 say the same thing, then I go with the 2. I am 61, had my knee replaced @ 55. I have had a total of 14 major surgeries in my life starting at 15, so I have had a lot of experience when it comes to opinions. I wish you the best.
