A friend of mine has TB, it really does worry me, im just wondering if alcohol+cigarette can make it worst?

well, cigarettes can't help. they need to be on medication!
Yes, it could. Smoking is probably what caused the TB in the first place. It couldn't be good for your friend. Alcohol is never good for anyone; especially in large amounts. If your friend is in late stages of TB (doesn't have long to live) a lot of people would let them have anything they want.
