My heart and thoughts go out to you and your friend. This is definitely a time for hard decision making.
The doctor is going to look at several things, but the treatment is ultimately going to be on the shoulders of your friend. This is no way in any order of the decision-making process, but there are some possibilities, such as placement of a feeding tube into the stomach or blood vessels, maybe radiation to shrink the cells of the throat, to even hospice referral if your friend thinks it's time.
Hold your friend's hand, either physically or emotionally. You don't have to say anything at all. When my father was dying, I remember just holding the phone to my mouth and breathing, because I couldn't do anything else, but I wouldn't let him think I would run away and not support him during his fight.

Well I am very sorry . Its interesting to watch what happens when oral intake is bad. Some family members want an IV started or a feeding tube. I do hope he doesnt want that as it simply drags out the process. I presume he has hospice care and there is no further treatment, right? I hope he can be kept comfortable. So sorry
Well, Huck. I hope you understand what Stage 4 means. The cancer has spread to distant organs.
If your friend can still think, and does not have Advance Directives, he should complete the forms now.
His doctor should make a referral to Hospice, unless your friend wants to be fed artificially and rescucitated when he stops breathing. These are critical issues, as you can tell.
