It depends how busy the MD is when you go and what kind of doctor he is.. i been to doctors where i had to wait over an hour and some where they see me as soon as I get there..

it depends on how long the wait is. sometimes you'll get right in and out in a half hour, and sometimes you'll be sitting in the waiting room for over an hour, twiddling your thumbs.
It depends on what you need to you. Since you haven't been seen in a while, it will probably include a new patient visit or a physical. Those are typically 45-60 mins. OTher appointments are typically 15 mins. You shouldn't need to book a double appointment. It should done by the receptionist to block you sufficant time. The waiting often comes from the doctor being behind. It happens often. I always plan on being at my doctors for at least 45 mins even for something simple like a cold.
