It sounds like you're talking about "Pectus Excavatum". This is a concavity in chest formed when parts of the rib grow and push the breast bone (sternum) in. It happens in about 1 out of 300 - 400 births, some much more pronounced than others. We're not exatly sure why some people get and some don't. There seems to be at least a little bit of heredity factor involved, at least in some cases.
I've seen some very obvious cases. One guy actually would occasionally fill his cavity up with water, or coke and just put a straw in it and drink while watching TV (more as a party gag than as a routine).
Truthfully, sometimes it can get bad enough to decrease the effeciency of the heart or, rarely, lungs. If so, surgery can be done to correct it. Most people want it corrected because of being self-conscious. The surgery to correct it is major surgery, and though most often it goes well, there can be some fairly major complications (of course, driving a car usually goes well, but it too can be fraught with "major complications"). If surgery is opted for I recommend that he postpones it until after his growth spurt. Usually best to wait until later teens, unless he is having heart/lung problems from the deformity.

Having those physical characteristics doesn't mean he's on steroids -- but if all of this happened in a relatively short amount of time, it means he could be.
