improper stretching and poor hydration- a good strech for the diaphragm is just to suck your gut in and hold it, and make sure to drink plenty of water the night before you jog- drinking while running (at least for me) ususally doesn't help much and just makes my stomach hurt. Drinking the night before allows your body the time to properly disperse the water.

it is because you are not breathing properly/enough, which causes lactic acid to enter your muscles, which is painful and causes a stitch.
To get rid of it you need to stop and breathe deeply, as you have an oxygen debt to your muscles and will need to send oxygen to your muscles before you regain your breath
From what I remember, this is caused by a lack of oxygen in your body, not breathing regularly enough, therefore your blood is not pumping fast enough. Develop a regular breathing pattern. Personally whenever I run I use roughly 70-74% of my lung capacity and I never get a stitch. A good gym should offer you the equipment used to measure lung capacity along with the treadmill, take them up on it because most people do not run in a progressive manner.

Basically, you are trying to run too fast for your current fitness level. So, slow down a bit, and maybe add some faster, short intervals into it until you can sustain the faster running for longer.
Explained simply, it's cramp. Your stomach goes in to a spasm - try not to exercise after eating.
