It sounds like she's large for her age - 4'8" in kindergarten? I'm not really sure whether she's too large or not, if so it's really up to her parents to help her, but you can let her see you eat good things when you're with her sister. Be kind to her - if she's an overeater, getting picked on won't make it better. And when you have a chance, be mentors to her! Take her outside, play with her, and encourage her to be active. She can get a lot of exercise just following you two around if she knows you'll let her.
I don't know any 5 yr old that is 4'8". Also, online BMI calculators aren't really meant for children, more for adults. If you are concerned and you are close to your friend and her family, you could mention it, but don't expect them to take it well. Alternatively, if you share a school nurse, you could mention your concerns to her and she could step in if necessary. The best thing you could do is encourage your friend's sister to play outside a lot. Play soccer and kickball with her and go rollerblading with her. What NOT to do?? Don't mention it to or in front of the little girl. If she is overweight, she will have enough problems without criticism from you. You can say "we should go outside and play. It's too nice to stay indoors," but don't mention weight or exercise.
