It means if you can't see light through it, don't drink it. Seriously, it means water, juice, soda, and broth. Beer and liquor doesn't count as a clear liquid. The idea is that there won't be anything in the stomach digesting, and very little in the intestines. Usually they only require it for 12 hours prior to surgery, but if your surgery is going to involve the abdomen it may be best if it's as empty as possible. Abdominal surgery usually leaves your insides rather wonky for a while afterwards. The last thing you would want to be is constipated as a result, or have something that would give you diarrhea and gas. Empty bellies rumble but they don't cause near as much grief post op.

I've never heard of such a thing but i would've thought the main factor would be water. Depending on what kind of surgery you're having will define what you have to eat and drink
Clear liquid is defined as anything you can see through in a glass. Avoid dairy and caffiene products. Chicken broth is okay and I have found my patients like to freeze their drinks and eat it with a spoon. This helps fool yourself that your eating!
