My wife and I have both used it. It was originally a prescription-only product marketed under the tradename "Xenical", but the generic, orlistat, is the same ingredient.
In a nutshell, here's how it works: Your body has several classes of enzymes which break down particular dietary products. Amylases break down starches, proteases break down proteins, and lipases break down fats (lipids). Because of the unique way that fat is transported into the body, they cannot be digested until the lipases break them down into the smallest of particles to be attached to other proteins for transport, absorption, and storage into adipose tissue (fat).
Orlistat interferes with the lipases and prevents them from doing their job on the fats and oils we ingest in our diets, hence they are not stored, and continue through the gastrointestinal tract and expelled from the body. This is why orlistat is called a "lipase inhibitor".
So to answer your question, yes, it works. To what degree depends on the individual, of course. A word of caution though: because you are not absorbing the fats, your body cannot do anything but excrete them. Therefore, your stools are incredibly oily, you may experience the fabled "oily discharge" with or without warning, which can be mighty embarassing. Your first couple of bowel movements may not look any different, but as you continue using it and your GI tract has been more or less flushed, don't be surprised to find a half-ounce or more of oil droplets in your bowl water.
You also tend to have a difficult time absorbing the important fats, such as the omega-3 fats found in fish as well as the important vitamins A,D,E, and K. For this reason, it's often recommended you take supplements, at least two hours before or after taking the orlistat. We find that bedtime is the best time for this.
Personally, I've lost 15 pounds which I can attribute to taking orlistat and have been on it for probably going on two months now. The side effects of "oil leaks" have either gone away or are more easily detected depending on what I eat. My wife and I eat better, since eating lower fat foods tends to decrease the side effects and gives us fewer surprises later on.
Also, there are several books out there which claim to be companions to the dietary recommendations made by the drug company, but are pretty much what I've explained and 98% dietary information for most foods. They range from $15 to $30 per book, depending on the title, but I would not recommend purchasing them, especially since there's a handy pamphlet in your bottle packaging explaining what I've already explained and more. Also, warehouse wholesale companies open to the public usually sell it for less money and more quantity than retail grocers or drugstores.
