My boyfriend has not been to the dentist for over 20 years. How bad is this?

Question: I've a new boyfriend. But I hate kissing him because, for some reason, he always tastes gross and has bad breath! I couldn't understand why. Then he said he'd not been to the dentist for 20 years! Is this why?! I tried to persuade him to go- but he says it's unnecessary as he cleans his teeth. How can I tell him tactfully that he tastes disgusting?! Don't want to dumb him- he's nice. I can't face just telling him straight- he's so sensitive. Any practical tips/ ideas for subtle hints?!
