My daughter wore one of those appliances several years ago. If I remember correctly, the "tube" thing in your hand should fit back into the other part of the appliance. Hers' would come apart if she opened her mouth to wide. It looks like a "shock absorber" when put back together. It should just slide back in. Hope this helps.
