It really depends on a number of factors (ie. oral bacteria count, oral hygiene, diet, genetics). Some studies show if a cavity is shallow enough (ie. <1/2 through enamel), there is a chance through proper remineralization (ie. fluoride therapy) and oral hygiene, the cavity has chance to actually shrink. This theory particulary applies to cavities between your teeth.

No one can say for certain if a cavity will "get bigger." A cavity is removed and filled with a filling material to remove and prevent the possibility of decay from spreading. No legitimate dentist can say with 100% absolute certainity all cavities will "grow."

The likelihood of spread of decay is much greater especially if an existing cavity is large, compared to a smaller one. It is for your health of course that all decay is removed, as all decay have a likelihood of growth and possibly necessitating the need for root canal and/or cown. But again, no one can claim ALL cavities will grow.
