How can i be sure i a second root canal isn't possible and i need to get tooth pulled?


Question:
Hi all. About 4 years ago I chipped a moler tooth and got a root canal and a crown.I was unhappy with the crown, I felt it didn't fit correctly. So it has bothered me on and off since then. It just get irritated for a day and goes away. I changed dentist. 4 days ago i up with a toothache out of this world(never experineced) a toothache. It is the thrid day and still pain so I went to go see a endodontist and he said that the xray looks like I have an absessed tooth because of the dark shading at the end of the root.He also said a second root canal won't help.That is what I don't understand.why do i have pain in a tooth that has had a rootcanal? He had a periodontist come in and he isn't sure so he wants to see me in a month. I do't want to lose tooth if not necessary.I also have pain in ear and back of head. So my question is how can I be sure it is my tooth and not something else like an ear infection making the pain so strong

You should consider having an apicoectomy, that is a simple surgery in which the tip of the root is removed and reshaped to obtain a better seal of the root canal. It works in the majority of cases, if root canal re-treatment is not indicated. Ask this to your endodontist, or an oral and maxillofacial surgeon. However, the prognosis (long term outcome) of a tooth that has been either retreated or surgerized, decreases, but it's worth a shot ( you only have your permanent teeth once )
