Why lidocaine infiltration did not have effect during dental manipulation for a healthy patient of 40 years.?

Another cause could be that there might have been inflammation around or near the site of injection/infiltration.
Inflammaion causes tissue acidity, and acid destroys some of the anesthetic. So it would have been rendered less effective.

But as the inflammation subsides in few days, the anes. will act.

Because lidocaine, as many other anestesic substances, is dependent to patient's toleration, which can change because of many aspects, such as if patient is nervous, if he/she slept well, if he/she has done drugs. And many others. It doesn't really have to do with he being healthy or not, so don't worry.

Lidocaine does nothing for me. I once had so much that I had double vision, and I was still not numb.

It's just your body; the same way that some people take Benadryl and it makes them sleepy; others take it and they're wired. Ask for Novacaine next time.
